Bone Marrow Concentrate (BMC) Stem Cell Therapy in Miami

Karli Health is led by Harvard-trained physician, researcher and published author David Karli, MD, MBA.

Bone marrow concentrate, or BMC, is a regenerative medicine and autologous stem cell therapy option that concentrates components from a patient’s own bone marrow blood, including platelets and multiple natural cell populations such as mesenchymal stem cells. At Karli Health, BMC is measured, processed and customized according to each patient’s diagnosis and treatment plan.

The Therapy

What Is Bone Marrow Concentrate?

Bone marrow concentrate is an autologous biologic, meaning it is made from material collected from your own body.

Bone marrow contains a complex mixture of platelets, growth factors, proteins and naturally occurring cell populations involved in tissue signaling, repair and maintenance. These include mesenchymal stem/stromal cells, hematopoietic cells and progenitor cells.

During a BMC procedure, a small amount of bone marrow blood is collected from the back of the hip bone and processed to concentrate selected components into a product that can be delivered to the physician’s intended treatment area.

Like platelet-rich plasma (PRP), BMC uses the patient’s own biology. The two preparations, however, are distinctly different.

Common Uses

How Does BMC Support Repair?

Bone marrow derived cells and proteins may support repair through multiple mechanisms. Some cells may attach to injured tissue and participate in tissue-specific repair activity. Others may release proteins and signals that communicate with cells in injured tissue and help coordinate the healing response.

Overview

What Makes BMC Different From PRP?

BMC is prepared from bone marrow blood and contains platelets plus a broader mixture of natural cell populations. Both are autologous and both vary in composition.

BMC is not automatically “stronger” or better than PRP. The appropriate option depends on the diagnosis, tissue, severity, evidence, treatment goals, procedural plan, and physician evaluation.

PRP is prepared from a standard blood draw from your arm, is platelet-focused and doesn’t contain stem cells.

Our Approach

What Makes Karli Health’s BMC Different?

Why is Karli Health’s BMC different?

Karli Health’s BMC process is built around the Karli Standard.

Karli Health’s specialized manual processing technique provides the capability to measure, concentrate and dose these cell populations so the final preparation can be aligned with the physician’s intended treatment target.

That’s a lot of technical talk. What does it mean?

Let us explain.

Many standard automated processing systems used to prepare BMC produce a fixed final product and do not provide the same ability to measure, customize and reconfirm the preparation and dose in real time. Even when a physician measures the finished product separately, the preparation produced by the device may not be adjustable if it does not meet the intended treatment target.

What you get is what you get.

At Karli Health, the preparation is measured before processing, customized according to the treatment plan, then remeasured and confirmed before treatment. Karli Health’s approach is designed to reduce the unknown: we customize and confirm the measured cellular profile of the BMC product so we know the cell dose, concentration and composition of the product being delivered.

Think about your injection as a medication. If you do not receive enough for your particular issue, the therapeutic response may be limited or ineffective. Have you ever taken too little of a medication and felt no effect, only to increase the dose and then experience a response? That is a simple analogy for why dose can matter in BMC, including the number and concentration of stem and other regenerative cell populations delivered.

Dr. Karli’s work over the past 20 years, including his work through Greyledge Technologies, has focused on measuring and refining regenerative biologic preparations and their dosing. Every Karli Health BMC preparation undergoes pre- and post-processing measurements, including total nucleated and mononuclear cells, platelets, white blood cells and red blood cells. The final volume and delivered dose are created, measured and confirmed for the physician’s specific treatment plan before injection.

Across Karli Health and other U.S. practices using Greyledge technology, the Greyledge Data Cloud has tracked more than 8,000 regenerative treatments as of August 2026, and that number grows every day as new treatments are added. Patient follow-up extends beyond two years and is ongoing. This multi-practice dataset allows physicians to compare treatment variables and dose with patient-reported outcomes and continue refining protocols for specific applications.

Karli Health knows what it is injecting.

If you are evaluating regenerative medicine physicians, ask whether they measure the biologic before and after processing, whether the final dose is known, and whether the preparation can be customized to the treatment plan. Ask what PRP or BMC processing technology they use and how they confirm what is actually being administered. Greyledge technology gives Karli Health the ability to measure, adjust and confirm its BMC preparations in real time.

That’s the Karli Standard in practice: know what was administered, know the dose, and connect it to the patient’s outcome over time.

That’s care that is measured, personal, and fit for you.

The Research

What Does Current Research Say About BMC?

Bone marrow concentrate has been studied across a range of orthopedic disease states and injuries, including knee osteoarthritis, cartilage-related disorders, rotator cuff and shoulder tendon injuries, low back pain, tennis elbow and other musculoskeletal conditions.

Recent systematic reviews and expert reviews report improvements in pain and functional outcomes following BMC treatment for knee osteoarthritis.

One consistent issue identified throughout the research is variability. BMC preparations can differ in how they are processed, their cellular composition and dose, injection protocols and patient selection. In other words, the term “BMC” does not necessarily describe the same biologic product from one treatment or study to another.

That variability matters.

At Karli Health, BMC is measured before and after processing so the physician knows what was produced and the dose being delivered. Treatment variables and patient-reported outcomes can then be tracked through the Greyledge Data Cloud and used to continue refining treatment protocols over time.

The Procedure

What Does the BMC Procedure Involve?

A BMC procedure begins with a small bone marrow blood aspiration from the back of the hip bone. The bone marrow blood is then measured, processed, concentrated and customized using Greyledge technology. The final preparation is remeasured to confirm the intended dose before being injected into the treatment area using ultrasound or fluoroscopic image guidance when appropriate for accurate placement. The entire process takes approximately 1–1.5 hours; we ask patients to plan to be with us for two hours.

Candidacy

Who May Be a Candidate for BMC?

Patients with certain joint, cartilage, tendon, ligament, sports-related or spine conditions who continue to experience pain, loss of function or limitations in activity may be candidates for BMC treatment.

BMC is not appropriate for every patient or every orthopedic condition. Candidacy depends on diagnosis, imaging findings, severity of the condition, symptoms and functional limitations, medical history, previous treatments, medications, overall health and treatment goals.

Karli Health’s approach is diagnosis-first. The goal is not to fit every patient into a stem cell procedure; it is to determine which treatment, if any, is medically appropriate for the individual patient.

Investment

BMC Cost and Insurance

BMC cost depends on the condition being treated and the complexity of the procedure. At Karli Health, joint BMC procedures generally range from $6,000–$9,000, depending on the condition, number of areas treated and procedural complexity. Spine BMC procedures generally range from $9,000–$15,000, depending on severity, disease state and the number and type of areas treated. Treatment may involve discs, facet joints, epidural regions and ligamentous structures when medically appropriate; treatment of spinal discs may also involve twilight anesthesia. A comprehensive orthopedic evaluation is necessary to determine the recommended regenerative treatment and specific procedural plan.

Regenerative medicine procedures, including BMC, are generally not covered by insurance, and all Karli Health services are self-pay.

Common Questions

Frequently Asked Questions About Bone Marrow Concentrate (BMC)

BMC stands for bone marrow concentrate. It is a regenerative medicine preparation made by concentrating components like platelets, white blood cells, mesenchymal stem cells, and important factors from a patient’s own bone marrow blood.

Yes. BMC is an autologous stem cell therapy because it uses a patient’s own bone marrow blood and contains naturally occurring cell populations that include mesenchymal stem/stromal cells and other progenitor cells. BMC is not a single-cell product; it contains multiple cell populations, platelets, proteins and signaling factors.

Yes. Bone marrow blood contains naturally occurring stem and progenitor cell populations, including mesenchymal stem/stromal cells. These cells represent only part of the complex mixture contained within BMC.

No. BMC is prepared from bone marrow blood and contains platelets along with bone-marrow-derived stem cells and other natural cell populations. PRP is made from a standard blood draw from your arm and is primarily platelet-focused. Both come from the patient’s own body, but they are different biologic preparations.

Bone marrow concentrate has been studied extensively for knee osteoarthritis. Clinical studies and recent reviews have reported improvements in pain and function in selected patients. Whether BMC is appropriate depends on the patient’s diagnosis, disease severity, imaging and physician evaluation.
